# 16 | The Gut-Hormone Connection
Most women know that the gut affects digestion. Fewer know that it also plays a direct role in how oestrogen moves through the body and that when this process goes wrong, the knock-on effects can include worsening PMS, irregular cycles, fertility challenges, and perimenopausal symptoms. The connection between gut health and hormonal health is one of the most important and least discussed areas of women's health and the research behind it has moved considerably in the last few years.
How the Gut Gets Involved in Your Hormones
Once oestrogen has done its job in the body, the liver processes it and sends it to the gut to be flushed out. In a healthy gut, most of it leaves the body as it should. However, certain gut bacteria produce an enzyme that can intercept this process — essentially unlocking a portion of that oestrogen and sending it back into circulation. In a well-balanced gut, this happens at a rate the body can handle. When gut bacteria are disrupted — through things like a low-fibre diet, chronic stress, antibiotic use or consistently poor sleep — this process can tip out of balance, leaving either too much or too little oestrogen circulating.
Too much recirculating oestrogen has been linked to heavier or more painful periods, worsening PMS, fibroids and endometriosis. Too little — which becomes more relevant as the ovaries naturally produce less oestrogen during perimenopause — can intensify symptoms like hot flushes, mood changes and disrupted sleep. The collective term for the gut bacteria involved in this process is the oestrobolome, and it is an area of active and growing research.
The Relationship Goes Both Ways
Just as gut health influences hormones, hormones influence the gut. Oestrogen receptors (aka docking points) are present throughout the digestive tract, which means that as oestrogen levels shift — across the menstrual cycle, during pregnancy, and during perimenopause — the gut responds. Research published in 2026 found that declining oestrogen during menopause is associated with reduced diversity in gut bacteria, a more permeable or ‘leaky’ gut lining and changes in how food moves through the digestive system. All of this can contribute not only to bloating and altered bowel habits, but to the broader symptom picture of perimenopause, including mood changes, hot flushes and poor sleep.
This is also why many women notice their digestion changes across the cycle. The bloating and sluggishness that often appear in the week before a period are partly driven by the rise and fall of progesterone, which slows down the digestive system. This is not coincidence because it is the gut responding to its hormonal environment.
What Actually Supports a Healthy Gut-Hormone Balance
Dietary fibre is consistently identified in the research as one of the most important factors. Fibre feeds the gut bacteria responsible for keeping the oestrogen recycling process in check and diets low in fibre are associated with imbalances that can raise circulating oestrogen levels. Studies suggest that women eating higher-fibre diets have measurably lower oestrogen levels — which for those managing oestrogen-driven conditions is clinically meaningful.
A wide variety of plant foods supports a more diverse and resilient gut environment overall. Fermented foods — yoghurt, kefir, kimchi, sauerkraut — contribute beneficial bacteria that help maintain that balance. Alcohol, by contrast, disrupts gut bacteria and has been shown to increase the amount of oestrogen recirculated through the system, which is one of the mechanisms behind the link between regular drinking and elevated oestrogen levels.
Chronic stress, poor sleep and antibiotics all affect the gut in ways that extend to hormonal health — which is another reason these factors matter beyond their more obvious effects.
Why This Is Worth Knowing
The gut-hormone relationship is not a wellness trend. It is a well-researched biological process with real relevance for women across all life stages. If you are managing PMS, endometriosis, fertility concerns or perimenopausal symptoms and your gut health has never come up in the conversation, it may be worth raising. The evidence is there — it has just not yet made its way consistently into routine care.
Your gut is doing considerably more work than it tends to get credit for and looking after it is one of the more evidence-based things you can do for your hormonal health.
